Hospice Care at St. Ann's Home
There may come a time when you need the very best skilled nursing care. That's when it's good to know there’s a place with a long history of caring for the most important people on earth. St. Ann’s Home is a 388-bed skilled nursing facility providing 24-hour on-site care. Our nursing home is set on 37 beautifully landscaped acres on Rochester’s northeast side. Outside, the gardens are flowering and fragrant. Inside, the floors shine. Security officers monitor the grounds and buildings 24 hours a day. House of John (Clifton Springs)
Home like setting for the terminally ill. All services at the House of John are free and there is no expectation of donations or contributions by residents or family members. Admission is not determined by religion, age, or ethnic background. If the potential resident is appropriate for the House and a bed is available, a bed will be offered, and arrangements can be made to move the resident to the House, usually within a few days. There is often a waiting list, but beds are offered to those with the most immediate or significant need. Please note the general admission criteria below: diagnosis of terminal illness with no expectation of recovery, a doctor's prognosis of three months or less life expectancy, private physician coverage, and no extraordinary care needs, such as IV's or feeding tubes.

Jewish Hospice Program (Jewish Senior Life)
Jewish Senior Life provides a specialized form of palliative care during the end stages of illness. Staffed by dedicated and specially-trained volunteers, the Jewish Hospice Program focuses on patient-centered care in your home, hospital, apartment, or another hospice setting to assist patients and families in making their loved one's remaining time both meaningful and comfortable. Our Jewish Hospice Program volunteers are specially trained in Jewish customs and rituals.

Journey Home (Greece)
Our goal is to provide guidance, support, comfort and pain management to individuals as they journey through the dying process, as well as provide guidance and support to the families of the dying individual. Based closely on the model of a family caring for a dying loved one at home, our staff and volunteers interact with the health care, social services and pastoral counseling professionals of certified home agencies so as to meet the individual palliative care needs of each resident.

The Pines of Peace is a residence for the terminally ill whose priorities are pain and symptom relief, along with emotional and spiritual support for both the resident and the family. Provide a comfortable, warm, alternate living arrangement for those individuals who have been deemed beyond curative care by their physician, two bed capacity is open, free of charge to anyone who meets the following criteria: need and physician's belief that an individual is in the last three months of his/her life.
