Adult Day Care: Social, recreational and/or rehabilitative services provided for persons who benefit from daytime supervision. An alternative between care in the home and in an institution. Refers to health support and rehabilitation services provided in the community to people who are unable to care for themselves independently during the day but are able to live at home at night.

Adult Day Health Care Program Medical Model (Jewish Senior Life)
The Adult Day Health Care Program at the Jewish Home of Rochester is a medical model program that serves adults with special physical and psychosocial needs. Encouraged by staff to increase their self-esteem, social interactions and independence, members who participate in the Adult Day Health Program are better able to remain with their caregivers in the community.
Brighter Day (M.M. Ewing Continuing Care Center)
The Brighter Day, founded in 1989, provides adults with a safe, friendly place to stay active while receiving personal services and medical supervision.The medical adult day program is designed for those who live alone or with a caregiver. In addition to medical supervision, we provide personal care, well-balanced meals and snacks, and the opportunity to spend time with others in a secure environment while loved ones enjoy peace of mind.
ElderONE (an affiliate of Rochester General Health System)
ElderONE, formerly Independent Living for Seniors, a Program of All-Inclusive Care for the Elderly (PACE), gives the frail elderly an alternative to nursing home placement with in home care. Each ElderONE participant is served by a team of dedicated in-home care providers. Consistent assignments help providers become familiar with participants over time. They can be on the lookout for even subtle changes to help us get ahead of the curve on medical, functional, and psycho-social problems that need our attention. That translates to peace of mind for both ElderONE participants and their families. Care team members include: primary care physicians and nurses; physicial, occupational and recreational therapists; social workers; home health aides; dieticians; and drivers. The ElderONE team works with the elderly and their caregivers to arrange for and monitor all the medical, social and daily living supportive services they need. We select services from the ElderONE network of providers. An individualized care plan is developed for each ElderONE participant by the interdisciplinary care team when they enter the program.
Fairport Baptist Home Adult Day Health
LifeTimes Adult Day Health Care, located in the city of Rochester on Plymouth Avenue adjacent to Plymouth Gardens, is operated by the Fairport Baptist Homes Caring Ministries. It is a day program which offers care to adults ages 18+ with medical needs and helps seniors maintain their independence through suppoprtive health and social services in a protective group setting.
Lifespan's Services for Older Adults w/ Intellectual & Developmental Disabilities
Lifespan has a combination of unique services for older adults with Intellectual and Developmental Disabilities including service coordination and two day programming options. Participants in all of Lifespan's services for older persons with Intellectual and Developmental Disabilities must be age 45 or older and have a documented (prior to age 22) developmental disability. Our services are overseen by New York State's Office of Mental Retardation and Developmental Disabilities (OMRDD).
Marian's House (an affiliate of Jewish Senior Life)
Marian's House is a contemporary home in a residential neighborhood that offers a retreat for those with memory impairment, and much needed respite for their caregivers. This day retreat is for people with early to mid-stage Alzheimer's and other related dementias, offering meals, activities, supervision and specialized programming in a warm, residential setting. The house is specially designed for both one-on-one interaction and group activities. Respite is also available on weekends only.
SeniorsFirst Adult Day Programs
The Seniorsfirst Adult Day Program is a social day program for those 55 and older that is specially designed for people who prefer to remain in their own homes or with relatives. The program provides an affordable option in a social setting for those who enjoy getting together with other seniors. Our clients may spend as little as a half day to as many as five days per week with us. You only pay for the time you spend here and you don't need to make a long-term commitment. You come when it is right for you or your loved one.
St. Ann's Home Connection Medical Day Service
Home Connection is a medical adult day service that provides quality care to individuals in a comfortable, stimulating, home-like environment. St. Ann's social workers develop personalized health care and recreation services plans for each person's health care needs, personal goals and requirements based on meetings with customers, their family caregivers and physicians' orders.
St. John's Home Day Break Adult Day Services
Day Break programs provide a safe, nurturing environment supervised by professional staff for the person who may need day time care to remain at home. Individual care plans, matching personal needs with appropriate services, are developed to promote independence and choice.
Unity Health System Medical Adult Day Services
For individuals with medically-complex needs, Unity Adult Day Services provide comprehensive medical, social, and rehabilitation services, in addition to therapeutic recreational activities.
