Our 60-bed Transitional Care Unit offers the most comprehensive post-acute services in Monroe County. Physically connected to the acute care services of Unity Hospital, the professional staff of the Transitional Care Unit utilizes an interdisciplinary approach in assisting patients regain functional independence to achieve a return to community living. Location Comments: Unity Living Center, a 120-bed state-of-the-art skilled nursing facility, is located on two recently renovated floors on the Unity St. Mary's Campus. Unity Living Center focuses on providing treatment and rehabilitative care for patients with medically complex needs, dementia, and behavioral challenges. Special services include short-term rehabilitation, pain control, IV therapy, peritoneal dialysis, access to hemodialysis, ventilator beds, and relaxation therapy to meet the needs of those with chronic diseases. | |
