ElderONE, formerly Independent Living for Seniors, a Program of All-Inclusive Care for the Elderly (PACE), gives the frail elderly an alternative to nursing home placement with in home care. Each ElderONE participant is served by a team of dedicated in-home care providers. Consistent assignments help providers become familiar with participants over time. They can be on the lookout for even subtle changes to help us get ahead of the curve on medical, functional, and psycho-social problems that need our attention. That translates to peace of mind for both ElderONE participants and their families. Care team members include: primary care physicians and nurses; physicial, occupational and recreational therapists; social workers; home health aides; dieticians; and drivers. The ElderONE team works with the elderly and their caregivers to arrange for and monitor all the medical, social and daily living supportive services they need. We select services from the ElderONE network of providers. An individualized care plan is developed for each ElderONE participant by the interdisciplinary care team when they enter the program.
