AIDS CareAIDS Care is the leading provider of HIV/AIDS services in Rochester and the Finger Lakes and was formed by the January 2010 merger of AIDS Community Health Center and AIDS Rochester. AIDS Care is the only organization in the Finger Lakes region with HIV/AIDS care and support services as our sole focus. Cancer Services Program of Monroe County (Formerly the Health Partnership)The Cancer Services Program of Monroe County (CSPMC) pays for breast, cervical, and colorectal cancer screenings for un-insured and under-insured women (ages 40- 64) and men (ages 50 -64). If a screening reveals a diagnosis of breast, cervical, colorectal or prostate cancer and the client has no insurance to cover treatment, the CSPMC will help the client apply for Medicaid Cancer Treatment Program coverage. Case management is also available for those needing additional support and assistance as they go through diagnosis and treatment. In addition, the CSPMC offers coverage for the human papilloma virus (HPV) vaccine for women ages 19 through 26. Appointments for breast exams, mammograms, pap test, and colorectal screenings can be made by calling the Cancer Services Program at 585-224-3070 or 1-877-293-0822. |

We have peer-led self-help support group meetings for those with the disorder (where family and friends are welcome). We are a local affiliate of the Depression and Bipolar Support Alliance (DBSA), which is headquartered in Chicago. Flower City Down Syndrome Network, Inc. (FCDSN)The Flower City Down Syndrome Network, Inc. (FCDSN) was begun in 1991 by a group of local families. Laura Khederian and Peggy Schoenfelder were two parents looking for a network of support for their children with Down syndrome. Finding that there were no groups in the Rochester area, they sent out a call for help from other support organizations and individuals and began their own network. Today, the FCDSN has a Board of Directors, sponsors 2 yearly Member Appreciation events, hosts 2 major fund raisers each year, collaborates with Strong Genetics to create information packets for parents that receive a positive diagnosis of Down syndrome for their child through amniocentesis or CVS, maintains 4 Social Groups for various ages of individuals with Down syndrome and their families, hosts a yearly Day-long Conference for FCDSN members, offers Cash Reimbursement programs for its members, and has hired a part-time paid staff person to assist in the organization of our many programs and our invaluable volunteers. |

Hickok Center for Brain InjuryThe mission of the Hickok Center is to provide a safe and supportive environment that encourages brain injury survivors to design and direct their own life's journey. Over the years, we have progressed into an agency that has 21 sites and employs 270 diverse employees. Juvenile Diabetes Research Foundation (Rochester New York Chapter)The Rochester New York chapter of the Juvenile Diabetes Research Foundation was founded in 1996. Gooley Hemophilia Center, located in Rochester New York, has been serving people who suffer from bleeding and blood-related disorders for over 50 years. Our Center was one of the first NHF chapters chartered outside of New York City, and the first free-standing comprehensive care clinic in the nation. Mended Hearts (Chapter 50 Rochester, New York)Mended Hearts provides help, support, and encouragement to heart disease patients and their families in the Greater Rochester Area through monthly meetings and visiting patients at Rochester General and Strong Memorial Hospitals. Mended Hearts, a national nonprofit organization affiliated with the American Heart Association, has been offering the gift of hope to heart disease patients, their families and caregivers for more than 50 years. Mended Hearts meets at the Henrietta Volunteer Ambulance Facility, 280 Calkins Rd, Henrietta, the third Monday of the month, at 7:15pm. For twenty eight years the self -help volunteer driven organization has been serving people of all cultures in Monroe and surrounding counties. We are an independent nonprofit (501c3) agency governed by a Board of Directors and affiliated with NAMI/New York State and NAMI National. We provide a forum for individuals and families to explore effective ways of coping with mental illness. Rochester/Finger Lakes Eye & Tissue BankRochester/Finger Lakes Eye & Tissue Bank recovers, processes, preserves and distributes eyes and other tissue for transplant, research and medical education; teaches about tissue and organ donation through its public and professional awareness programs; funds transplant-related medical research; and maintains the Regional Donor Pledge Registry. Trained and certified RETB/FLETB tissue bank staff recover ocular, musculoskeletal, skin and cardiovascular tissues. Tissue bank coordinators then process, preserve and distribute this tissue for transplant, research and medical education. |

The Mocha ProjectThe MOCHA Project (Men of Color Health Awareness) is a peer driven initiative which has been in existence since December 1996. MOCHA provides HIV/AIDS education/outreach, and community development for men of color who are identified as gay, bi-sexual or men who have sex with men (MSM) or young men who have sex with men (YMSM). Currently, several funding sources support MOCHA Project's programs to provide education, prevention & intervention to youth who are MSM's of color and may be at risk for contracting the HIV virus. |

Threshold Center at Community PlaceThreshold Center at Community Place includes a Health Center that provides a unique opportunity for adolescents aged 12-26 to receive primary and reproductive care in a confidential, comfortable setting. Transplant Awareness Organization of Greater RochesterTAO is an association of individuals that have undergone transplant surgery. TAO members provide support for other transplant recipients, candidates and their families. By sharing their personal experiences, these former patients complement the counseling provided by physicians, social workers and other health care professionals by offering special understanding that comes only from experiencing a transplant firsthand. In addition to offering support, TAO members also participate in special events throughout the year to promote organ donor awareness. Four months later in March of 2003, we became a local chapter of the Depression and Bipolar Support Alliance (DBSA), a national organization dedicated to improving the lives of those with mood disorders. The group's name at that time, and for the next five years, was DBSA Parents with Bipolar Children, Rochester NY. Under our affiliation with DBSA, we became a corporation and received status as a 501 (c3) organization. |
