Coalition to Prevent Lead Poisoning (CPLP)Founded in 2000 as the Lead Free Rochester Coalition, the Coalition to Prevent Lead Poisoning (CPLP) is an education and advocacy organization composed of nearly 100 individuals and community organizations dedicated to eliminating childhood lead poisoning in Monroe County. Our mission is to provide leadership and advocacy to empower the community and its residents to prevent the lead poisoning of children by creating an environment that is free of lead hazards, facilitates the creation of a system that protects children, creates jobs, and enhances property values. |

Finger Lakes HIV Care Network (FLHCN)The mission of the Finger Lakes HIV Care Network (FLHCN) is to recognize, evaluate, and respond to current and emerging issues related to HIV/AIDS in our nine county region in upstate New York. FLHCN accomplishes this mission by supporting collaborative forums which bring together individuals and agencies in order to develop comprehensive plans that address identified needs. FLHCN values inclusivity, open communication, respect, and empowerment as tools to accomplish its mission. |

HEALTH ACTION, Priorities for Monroe CountyThe goal of HEALTH ACTION is to improve the health of the citizens of Monroe County by aligning community resources to focus on selected priorities for action. HEALTH ACTION facilitates collaboration and cooperation among providers, agencies and businesses to benefit the community by concentrating resources where they are most likely to impact health status. A healthy community is one in which the objective measures of health status are at optimal levels and when there exists a commitment to health at all levels of the community. |

Healthi Kids InitiativeWhat is the Healthi Kids initiative? A community-based coalition advocating for five policy and practice changes: 1.Better school food, community standards Eliminating the availability of school foods that compete with the national school breakfast and lunch program, and mandating the development and execution of nutritional standards so all food available on school campuses is consistent with a set of community standards. 2.Safer, more accessible play areas Improving the safety of, and access to, recreational facilities, bike trails, parks and green spaces, while expanding after-hour access to schools and promoting safe play. 3.Healthier food standards at pre-school and childcare centers Developing and implementing nutrition standards for pre-schools, childcare centers and school-age childcare programs. Available food and drinks must comply with Dietary Guidelines for Americans or equivalent community standards. 4. At least 45 minutes of in-school physical activity Requiring schools to provide a minimum of 45 minutes of moderate to intense physical activity within the school day for K-12 grade students. |

Monroe County ACCESS (Achieving Culturally Competent and Effective Services and Supports)Monroe County ACCESS (Achieving Culturally Competent and Effective Services and Supports) began in 2005 and is an initiative of the Office of Mental Health within the Department of Human Services of Monroe County. ACCESS is funded by a SAMHSA (Substance Abuse and Mental Health Services Administration) grant, which is also referred to as the System of Care grant. Monroe County ACCESS seeks to facilitate and improve cross-systems collaboration and improve the coordination of mental health supports and services to emotionally and behaviorally challenged youth and their families. This approach to changing how child-serving systems work and work better together is referred to as a System of Care for children's mental health. |

The Pediatric Links with the Community ProgramThe Pediatric Links with the Community Program, or PLC as it is best known, is an innovative partnership between the University of Rochester Department of Pediatrics, Medicine-Pediatrics, Family Medicine, School of Nursing, Community and Preventive Medicine, Psychology and Rochester community-based agencies that serve children and their families. Our Mission: To inspire and enable health care professionals to participate in community-based partnerships, evidence-based programs, and advocacy to improve the health of children and families. |
