Coalition to Prevent Lead Poisoning (CPLP) Founded in 2000 as the Lead Free Rochester Coalition, the Coalition to Prevent Lead Poisoning (CPLP) is an education and advocacy organization composed of nearly 100 individuals and community organizations dedicated to eliminating childhood lead poisoning in Monroe County. Our mission is to provide leadership and advocacy to empower the community and its residents to prevent the lead poisoning of children by creating an environment that is free of lead hazards, facilitates the creation of a system that protects children, creates jobs, and enhances property values. |

Eat Well Live Well for Healthy Blood Pressure Eat Well Live Well for Healthy Blood Pressure is a coalition of more than 50 organizations led by the Rochester Business Alliance and the Finger Lakes Health Systems Agency, whose aim is to improve the health of the Rochester region by reducing the serious impact of high blood pressure in our community. |

HEALTH ACTION, Priorities for Monroe County The goal of HEALTH ACTION is to improve the health of the citizens of Monroe County by aligning community resources to focus on selected priorities for action. HEALTH ACTION facilitates collaboration and cooperation among providers, agencies and businesses to benefit the community by concentrating resources where they are most likely to impact health status. A healthy community is one in which the objective measures of health status are at optimal levels and when there exists a commitment to health at all levels of the community. |

HEART (Health Engagement and Action for Rochester's Transformation) The greater Rochester community was awarded a Community Transformation Grant by the Centers for Disease Control and Prevention (CDC) in recognition of this community's spirit of collaboration and shared vision for community health improvement. The University of Rochester Medical Center, the Monroe County Department of Public Health and numerous community partners were awarded a five-year, $3.6 million grant to fund HEART, an initiative to improve the health of area residents through policy and environmental changes. HEART focuses on strategies that support tobacco free living, healthy eating and physical activity, hypertension and diabetes prevention. |

Injury Free Coalition for Kids of Rochester Unintentional injury is the leading cause of death in children less than 18 years of age in Monroe County. As a result, Monroe County has made reducing unintentional childhood injuries a health priority. The Injury Free Coalition for Kids of Rochester (IFCKR) is an injury prevention program whose goal is to address this health priority in our community. Centered at the University of Rochester Medical Center, IFCKR's strength is working with community groups, governmental agencies and others to identify community concerns, develop plans to address those concerns, and implement prevention activities. By relying on the strengths of our coalition members, IFCKR is able to address community needs for injury prevention programs. |

Monroe County ACCESS (Achieving Culturally Competent and Effective Services and Supports) Monroe County ACCESS (Achieving Culturally Competent and Effective Services and Supports) began in 2005 and is an initiative of the Office of Mental Health within the Department of Human Services of Monroe County. ACCESS is funded by a SAMHSA (Substance Abuse and Mental Health Services Administration) grant, which is also referred to as the System of Care grant. Monroe County ACCESS seeks to facilitate and improve cross-systems collaboration and improve the coordination of mental health supports and services to emotionally and behaviorally challenged youth and their families. This approach to changing how child-serving systems work and work better together is referred to as a System of Care for children's mental health. |

The Pediatric Links with the Community Program The Pediatric Links with the Community Program, or PLC as it is best known, is an innovative partnership between the University of Rochester Department of Pediatrics, Medicine-Pediatrics, Family Medicine, School of Nursing, Community and Preventive Medicine, Psychology and Rochester community-based agencies that serve children and their families. Our Mission: To inspire and enable health care professionals to participate in community-based partnerships, evidence-based programs, and advocacy to improve the health of children and families. |
